Registering with the Practice

In order to receive care from a GP when you want it (rather than in an emergency) you must register with a Practice. To register at the practice please ask reception for our registration forms. The receptionist will hand you two forms (family doctor services registration form (GMS1 Form), and a patient questionnaire). Simply complete the forms and return them to reception. Please specify to reception if you wish to register as a temporary resident as the forms you need to complete are slightly different. If you require assistance filling in these forms please ask a member of staff. Once you have registered with us, the local Health Board will send us your medical records from your previous GP. Children under 16 need to be registered by their parents.

Our main area of patients registering with our practice falls under the villages of New Tredegar; Phillipstown; Deri; Fochriw; Aberbargoed and Bargoed. However, we also have patients in outer lying areas such as Rhymney; Pontlottyn; Blackwood; Fleur-de-Lys and Hengoed.
