Our District nurses are able to visit you at home if you are housebound.

They are able to provide care which includes: - support for patients and their carers during periods of illness, wound care including post-operative care, assessment on continence and catheterisation. They also offer ‘flu / pneumonia’ injections to all housebound patients.

The aims of the District nursing team are:

· To teach you and /or your carer how best to manage your care and to promote your independence.

· To regularly evaluates your care needs and the frequency of your nursing visits.

· To involve you and your carers in all decisions.

· To obtain informed consent from you for all procedures performed – as per trust policy.

· To reduce the number of unnecessary hospital admissions by providing the primary health care team with an alternative that enables people to stay in their own homes.

· To support safe discharge of patients from the hospital who require acute intervention that can be provided safely and effectively within their own home or other community environment.
